Numbness of the hand

Description

Онемение кисти отмечается при поражении периферических нервов, нервных сплетений и корешков, некоторых других неврологических заболеваниях, травмах, заболеваниях сосудов. Возникает постоянно или беспокоит периодически, сочетается с болями, парестезией, слабостью, атрофией мышц. Этиология онемения определяется с учетом анамнеза, данных общего и неврологического обследования, РПИ, рентгенографии, МРТ, КТ, лабораторных исследований. Лечение включает обезболивающие, лекарства для улучшения кровообращения и снижения мышечного тонуса, витамины и физиотерапию.

Reasons

Кратковременное онемение сразу после пробуждения вызвано сдавлением сосудов и нервов из-за неудобного положения руки. Сопровождается ощущением покалывания, «мурашек по коже», «непослушания» члена. Возникает у людей, которые кладут руку под тело или под подушку, забрасывают конечность над головой. Исчезает через несколько минут после смены положения тела.
Онемение кисти определяется невритами инфекционной этиологии, невропатиями из-за сдавления нервных стволов разрастаниями костей, опухолями, воспалительными инфильтратами:
• Лучевой нерв. Гипестезия обнаруживается на тыльной стороне плеча, на тыльной стороне предплечья.
• Локтевой нерв. Наблюдается нарушение чувствительности локтевой стороны ладони.
• Срединный нерв. Страдает лучевая половина ладони.
Для них характерны симметричные поражения кисти. Вначале процесс затрагивает преимущественно дистальные отделы конечности, затем онемение, парестезия и другие симптомы распространяются в проксимальном направлении. Полинейропатии возникают при алкоголизме, тяжелых соматических заболеваниях, онкологических процессах, некоторых инфекциях, вибрационной болезни, экзогенной интоксикации. Иногда они развиваются во время беременности.
Поражение верхнего пучка плечевого сплетения проявляется онемением внешней стороны предплечья и плеча, нижнего пучка - гипестезией внутренней стороны плеча, со стороны переднего плеча, кисти. Это связано со слабостью мышц того же места. При полном поражении плечевого сплетения выявляется снижение или потеря чувствительности как с внешней, так и с внутренней стороны руки. Причина развития плексита плеча:
• Родовая травма с образованием акушерского паралича.
• Травматическая травма. Вывих плеча, перелом ключицы, травма связочного аппарата плечевого сустава.
• Компрессия плечевого сплетения. Аневризма, гематома, шейное ребро, опухоль.
• Инфекционное заболевание. Герпес, цитомегаловирус, бруцеллез, туберкулез, сифилис.
• Дисметаболические расстройства. Сахарный диабет, подагра, диспротеинемия, нарушения обмена веществ.
• Ятрогенные повреждения при операциях и манипуляциях.
Синдром скалена возникает в результате сдавления ветвей плечевого сплетения, артерий и подключичных вен наклонными мышцами. Характерной чертой патологии является внезапное появление симптомов. Боль в шее, руке, голове дополняет мышечную слабость, парестезию, бледность кожи рук, слабость или отсутствие пульса. Площадь онемения зависит от поражения определенных ветвей сплетения.
Наиболее частой причиной синдрома наклона является шейный остеохондроз. Сдавливание мышц также развивается при искривлении позвоночника, после травм, на фоне врожденных аномалий, подъема тяжестей и значительного набора веса.
Онемение руки развивается при компрессии, травматическом повреждении, воспалении корешков спинного мозга на уровне нижних шейных и верхнегрудных позвонков. Причины - остеохондроз, межпозвонковая грыжа, спондилез. Симптом можно обнаружить при травмах позвоночника, спондилолистезе, остеомиелите позвоночника, менингите позвоночника. Иногда корешковый синдром формируется на фоне невромы корня, опухоли спинного мозга. Тема зависит от затронутого корня:
• с5 - онемение на внешней стороне плеча;
• С6 - нарушена чувствительность внешней поверхности предплечья;
• С7 - страдает задняя поверхность плеча и предплечья;
• С8 - по внутренней поверхности предплечья обнаруживается гипестезия;
• Т1-Т2 - возможно онемение в руках.
Дистрофические изменения спинного мозга сопровождаются сенсорными нарушениями ниже уровня повреждения. Степень изменения зависит от степени поражения. Гиестезия или потеря чувствительности в руках вызвана миелопатиями следующего генеза:
• Спондилоген и дискоген. Остеохондроз, спондилез, спондилоартроз, инволютивный спондилолистез, протрузия диска, межпозвонковая грыжа.
• Ишемический. Преходящие нарушения спинномозгового кровообращения, нарушения мозгового кровообращения.
• Посттравматический. Сотрясение мозга, ушиб спинного мозга, переломы с травмой спинного мозга, другие травмы спинного мозга.
• Карциноматозный. Паранеопластические процессы при лимфогранулематозе, лейкозах, раке легких и других онкологических процессах.
• Инфекционный. Нейро СПИД, нейросифилис, болезнь Лайма.
• Другое. Токсический, лучевой (на фоне лучевой терапии), метаболический (при эндокринных заболеваниях, нарушениях обмена веществ).
Онемение и покалывание - наиболее распространенный тип ауры мигрени. Начинается от кончиков пальцев, распространяется на всю руку, половину шеи. Затем расстройства исчезают и сменяются головными болями. Онемение, покалывание, жжение при сенестопатиях характеризуются самой разнообразной локализацией в сочетании с другими часто претенциозными жалобами. Ощущения мигрирующие, периодические или постоянные, могут распространяться на различные участки тела, включая руки.
Болезнь Такаясу проявляется онемением, слабостью, болью в руках в результате воспалительного поражения аорты и крупных артерий. В зависимости от места воспаления может поражаться одна или обе конечности. Внешний осмотр показывает ослабление или отсутствие пульсации, побеление кистей рук. Возможны обмороки, головокружение, нарушение зрения, проблемы с сердцем.
В отличие от других сосудистых обструктивных заболеваний, при тромбоэмболическом васкулите могут поражаться не только нижние, но и верхние конечности. Появляются боль, онемение кистей и предплечий, бледность, цианоз дистальных отделов конечностей, отсутствие пульса, образование болезненных узелков на пальцах рук, трофические нарушения.
Другие состояния, которые могут быть связаны с онемением руки, включают:
• Заболевания, вызываемые ОРВ. Бурсит плечевого сустава, воспаление плечево-лопатного сустава.
• Сердечно-сосудистая патология. Ишемическая болезнь сердца, инфаркт миокарда.
• Психические расстройства. Паническое расстройство, истерия.

Этиологию онемения рук устанавливает невролог. При сосудистой патологии необходима консультация сосудистого хирурга. Специалист изучает анамнез заболевания, оценивает состояние конечности, пульс на артериях, выявляет отек, признаки воспаления и другие изменения. Неврологическое обследование включает исследование чувствительности, рефлексов и силы мышц. По результатам назначаются:
• Электрофизиологические исследования. Eng, EMG, вызванные потенциалы проводятся для дифференциации поражения нервов, мышц и структур центральной нервной системы, для уточнения локализации, степени поражения нервного ствола.
• Рентгенография. Его проводят при травмах, подозрении на сдавление нервов костными разрастаниями, новообразованиями. Визуализирует переломы, вывихи, остеофиты, деформации костей.
• Исследование кровеносных сосудов. Информативно при синдроме лестничной мышцы, болезни Такаясу, облитерирующем тромбангиите. Для определения локализации и степени окклюзии может быть проведено УЗИ сосудов, ангиография и аортография.
• Другие техники визуализации. Компьютерная томография и магнитно-резонансная томография выполняются при травмах и травмах. Их назначают в случае неоднозначных результатов фундаментальных исследований, планирования хирургических вмешательств.
• Лабораторные тесты. Рекомендуется для подтверждения инфекционного и дисметаболического происхождения полинейропатии, диагностики болезни Такаясу.

При онемении из-за неврологических заболеваний необходимо исключить нагрузки, провоцирующие усиление симптомов, делать регулярные перерывы для отдыха рук. Пациентам назначают витамины группы В, мочегонные средства для устранения отеков. По показаниям план лечения дополняется хондропротекторами. При хронической боли могут потребоваться антидепрессанты. В список немедикаментозных методов входят фонофорез, УВЧ, гидротерапия, лечебные грязи, озокеритовые и парафиновые аппликации. Важная роль отводится массажу, ЛФК, мануальной терапии, рефлексотерапии.
При болезни Такаясу рекомендуются иммунодепрессанты, антиагреганты и антикоагулянты. Проводятся плазменная каскадная фильтрация, лимфоцитаферез, плазмаферез. При тромбангите используются спазмолитики, НПВП, средства, улучшающие реологические свойства крови и активирующие местный обмен веществ. Ориентируются на ВЛОК, гипербарическую оксигенацию, гемосорбцию.
Тактика хирургического вмешательства определяется характером патологии, вызвавшей онемение рук. Невралгия, вызванная сдавлением нерва опухолью, рубцеванием, фиброзом или разрастанием костей, считается показателем невролиза, удаления опухоли или декомпрессионной операции.
При патологиях позвоночника используются дискэктомия, коррекция сколиоза, операции, направленные на устранение стеноза позвоночного канала и нестабильности позвоночника. Устранение причины появления плечевого плексита проводится путем удаления шейных ребер, иссечения опухолей и аневризм, коррекции положения отломков при переломах.
