Description

Боль в спине свидетельствует о заболевании или поражении позвоночного столба, нервных структур, мягких тканей, внутренних органов, наблюдается при лихорадочных состояниях различного происхождения. Бывает резким, тупым, постоянным, периодическим, жгучим, стреляющим, разрывным. Иногда связано с физическими нагрузками, сменой погоды. Патологии, проявляющиеся болями в спине, диагностируются на основании жалоб, анамнеза, данных обследования и дополнительных исследований: рентгенографии, МРТ, КТ. Перед постановкой диагноза необходим отдых, иногда допустимо использование мазей, прием анальгетиков.

Почему болит поясница.

Заболевания позвоночника.

Врожденные дефекты и приобретенные заболевания позвоночника - частые причины болей в спине. Характер болевого синдрома различен. Боли, вызванные непосредственно патологическим процессом, чаще периодические, местные, ноющие или тянущие, связаны со временем, физическими нагрузками и нахождением в неудобном положении.
Из-за мышечных спазмов возникает люмбаго (люмбаго) - резкая боль, сопровождающаяся ограничением движений. При многих патологиях позвоночника наблюдается люмбоишиалгия - ноющая, жгучая или пульсирующая боль в пояснице, иррадирующая по задней поверхности бедра. Болезненные ощущения часто встречаются в составе корешкового синдрома. Их можно обнаружить при следующих заболеваниях позвоночника:
• Дегенеративные патологии. Остеохондроз, протрузия межпозвонкового диска, межпозвонковая грыжа, спондилез, спондилоартроз.
• Врожденные аномалии. Сакрализация, люмбаризация.
• Полиэтиологические условия. Спондилолиз, спондилолистез.
• Сосудистые заболевания. Нарушения спинного кровообращения.
• Прочие болезни. Болезнь леса.
• Вторичное повреждение нервных структур. Воспаление пояснично-крестцовых корешков, воспаление пояснично-крестцового сплетения, миелопатии различного происхождения.

Diagnostics

Первичный диагноз ставит ортопед-травматолог. При наличии неврологической симптоматики больной осматривается неврологом. Врач опрашивает пациента, проводит объективное обследование. По показаниям назначаются консультации хирурга, ревматолога, уролога и других специалистов. Программа диагностики может включать:
• Неврологическое обследование. Во время исследования специалист оценивает рефлексы, силу мышц, координацию движений, глубокую и поверхностную чувствительность.
• Рентгенография. На рентгенограммах поясничного отдела позвоночника видны переломы, уменьшение высоты межпозвонковых дисков, другие дегенеративные изменения, новообразования, признаки воспаления, спондилолистеза. При необходимости стандартные рентгенологические снимки дополняются функциональными обследованиями.
• Другие методы нейровизуализации. Компьютерная томография и МРТ используются для уточнения рентгеновских данных. Компьютерная томография детально исследует структуру твердых структур, МРТ исследует состояние связок и межпозвонковых дисков. Чтобы исключить стеноз, проводится миелография.
• Функциональные тесты. Состояние мышечной и нервной проводимости оценивается с помощью электромиографии, электронной нейрографии и исследований вызванного потенциала.
• Лабораторные тесты. Для подтверждения инфекционной природы заболевания, для определения возбудителя проводятся анализы крови и микробиологические исследования. Серологические тесты используются для выявления нейроинфекций.
По показаниям проводится УЗИ почек, простаты, органов малого таза, мочи, УЗИ брюшной аорты и другие анализы.
Мануальная терапия.

Classification

В случае травматического повреждения позвоночника пациента следует поместить на щит и немедленно доставить в медицинское учреждение. При нетравматической боли следует снизить нагрузку на спину, оптимизировать положение тела во время работы и отдыха. Острый болевой синдром - показание к неврологу.
Возможен разовый прием анальгетиков до осмотра специалиста. При болях в пояснице, люмбоишиалгии, вызванной ранее диагностированными дегенеративными заболеваниями позвоночника, допустимо применение местных согревающих и обезболивающих препаратов. При подозрении на инфекционный процесс местные средства не показаны.

Treatment

Основа лечения - физиотерапия и медикаментозная терапия. Пациенту назначен режим защиты. Используются следующие методы:
• НПВП. Эффективен при острой и хронической боли в мышцах и позвоночнике. Применяется в виде таблеток, средств местного действия.
• Нейротропные витамины. Пациентам вводят витамины группы В, которые усиливают действие препаратов других групп, способствуя уменьшению боли.
• Местные анестетики. При стойкой и острой боли проводят лечебный блок с анестетиками. Для улучшения результата лечения обезболивающие комбинируют с глюкокортикостероидами.
• Физиотерапия. Используются ультразвук, магнитотерапия, чрескожная электростимуляция, лазерная терапия и лекарственный электрофорез. Возможно назначение массажа, мануальной терапии, акупунктуры.
Учитывая особенности патологии, проводятся следующие хирургические вмешательства:
• Нестабильность: межсоматический спондилодез, транспедикулярная фиксация, фиксация пластиной.
• Опухоли, остеопороз, остеомиелит, туберкулез: секвестрэктомия, вертебропластика, кифопластика, корпорэктомия.
• Межпозвоночная грыжа: дискэктомия, микродискэктомия, нуклеопластика.
• Сужение позвоночного канала: ламинэктомия, фасетэктомия, декомпрессия пункционного диска.
В послеоперационном периоде назначают анальгетики, антибиотики. Реабилитационные мероприятия включают ЛФК, массаж, физиотерапию.
