Description

Чихание в норме возникает под воздействием химических или физических раздражителей слизистой оболочки носа, при этом в сухом помещении беременность. Патологический симптом возникает при остром и хроническом рините, аллергическом рините, синусите и полипах носа. Диагностика включает переднюю и заднюю риноскопию, рентген придаточных пазух носа, микробиологический и цитологический анализ мазка из носа. Местное лечение проводится каплями в нос, содержащими гормоны, антибиотики, иммуномодуляторы. Лечение дополняется антигистаминными и нестероидными противовоспалительными препаратами, аллерген-специфической иммунотерапией и хирургическими методами.

Чихание - это естественная защитная реакция дыхательных путей на раздражители. Симптом вызван проникновением частиц пыли в нос и вдыханием резких запахов (сигаретный дым, парфюмерия). Люди чихают, попадая на холодный воздух из теплого помещения, и наоборот. У 20-25% людей появляется «легкий рефлекс чихания» - при взгляде на яркий свет или солнце. Раздражение нервных рецепторов при чистке ушей ватными палочками также стимулирует чихание.
Симптом часто наблюдается при длительном пребывании на сухом воздухе. Особенно это заметно зимой, когда радиаторы работают на обогрев помещения после установки кондиционера. Человек регулярно чихает, ощущает постоянную сухость и дискомфорт в носу. При этом выделяется небольшое количество вязкой слизи. Иногда пересыхание слизистой оболочки носа приводит к повреждению капилляров и в выделениях становятся заметными прожилки крови.

Ринит беременных.

Состояние обычно проявляется в третьем триместре. Чихание вызывается гормональными изменениями в организме женщины, которые влияют на тонус сосудов слизистой оболочки носа. Симптом наблюдается периодически, дополняется заложенностью носа и небольшими слизистыми выделениями. Чихание возникает на фоне удовлетворительного общего состояния женщины, без типичных признаков респираторной инфекции.
• Острый ринит. Частое и повторяющееся чихание с выделением жидкой прозрачной слизи. Симптом может усугубиться промыванием носа, закапыванием лекарственных капель. Через 2-3 дня чихание появляется гораздо реже, только при скоплении слизи в носу.
• Вазомоторный ринит. Характеризуется периодическими эпизодами чихания (10-20 раз), во время которых выделяется обильная прозрачная слизь. Симптомы ухудшаются после ночного сна, при приеме пищи, при контакте с холодным или сухим воздухом. Вне пароксизма чихания нет, но пациента беспокоит заложенность носа.
• Гипертрофический ринит. При этой форме ринита чихание развивается в основном по утрам, когда в носу скапливаются липкие выделения. Пациент несколько раз чихает, чтобы избавиться от мешающей слизи. Выделения часто настолько густые, что их необходимо удалить вручную или промыть.
• Озена. Чихание наблюдается на начальных стадиях этой формы атрофического ринита. Из носовой полости выделяются неприятно пахнущая слизь или гной, засохшие желтые или коричневые корки. Постепенно пропадает защитный рефлекс, связанный с нарушением функции нервных окончаний носа.

Врач-оториноларинголог старается выяснить причины чихания. На первой консультации врач спрашивает пациента, как давно начались симптомы, проводилось ли лечение дома. При объективном осмотре специалист проявляет внимание к наличию высыпаний на коже, прислушивается к легким, чтобы вовремя выявить распространение заболевания на нижние дыхательные пути. Для диагностики используются лабораторные и инструментальные методы:
• Риноскопия. Самый информативный метод, с помощью которого врач исследует слизистую носа. О патологии, проявляющейся чиханием, свидетельствует гиперемия или цианоз стенок носовых полостей, отечность, наличие гнойных или кровянистых корок. При риноскопии носовые полипы визуализируются как гроздевидные разрастания.
• Рентгенография. Рентген придаточных пазух носа (пазух) необходим для исключения синусита. При хроническом чихающем рините на снимке виден пристеночный отек слизистой оболочки гайморовой пазухи. Компьютерная томография придаточных пазух носа рекомендуется для оценки размера и расположения полипов.
• Микробиологические исследования. При чихании обязательно возьмите мазок со слизистой носа или образец секрета для анализа на вирусы и бактерии. Цитологическое исследование биоматериала (риноцитограмма) необходимо для определения количества и соотношения эозинофилов, нейтрофилов, лимфоцитов.
• Аллергические тесты. После ремиссии сенной лихорадки или атопической бронхиальной астмы проводят кожные пробы на наиболее распространенные аллергены. Методика используется для выявления провоцирующих факторов и по возможности исключить их влияние на организм пациента.
Риноэндоскопия.

Treatment

Местное лечение выбирается для уменьшения чихания и других назальных симптомов. Существует несколько видов капель для носа, при выборе которых учитываются причины ринита. Противомикробные препараты рекомендованы пациентам с заболеванием бактериальной этиологии, при вирусных заболеваниях - иммуномодуляторы, интерфероны. Чтобы быстро справиться с аллергическим чиханием, существуют спреи для носа с кортикостероидными гормонами.
Системные препараты назначают для лечения основного заболевания, вызывающего рефлекс чихания. При ОРВИ используются жаропонижающие препараты, иммуностимуляторы, при гриппе - специфические противовирусные препараты. Антигистаминные препараты и ингибиторы лейкотриенов показаны для лечения аллергических заболеваний. Если их эффект недостаточен, прибегают к аллергенспецифической иммунотерапии (АСИТ).
При хроническом рините используется назальный электрофорез с кальцием, дифенгидрамином или витаминами. Методика уменьшает сухость и отечность слизистой оболочки, нормализует тонус сосудов полости носа. Из направлений физиотерапии при чихании лучшими являются УВЧ, ОКУФ терапия, парафинотерапия и иглоукалывание. В редких случаях лечение дополняется гомеопатическими препаратами.
Хирургическое вмешательство требуется при хронических формах насморка (особенно при гипертрофической форме), когда восстановить дыхание лекарствами невозможно. ЛОР-хирург применяет лазерную вазотомию, дезинтеграцию носовых ходов ультразвуком или классическую подслизистую конхотомию. С озером показана обратная операция - уменьшение просвета носовых ходов.

Никаких специальных лечебных мероприятий при банальном рините, вызванном ОРВИ, не требуется. Для промывания полости носа необходимо использовать физиологические растворы для удаления частичек возбудителя и экссудата, а также для увлажнения слизистой оболочки. При ощущении запора и затруднении дыхания нельзя использовать сосудосуживающие капли более 7 дней, строго соблюдая инструкцию.
Чтобы уменьшить аллергическое чихание, следует ограничить контакт с раздражающими факторами: меньше находиться на улице в период цветения деревьев, регулярно проводить влажную уборку для удаления пыли. При обострении аллергического ринита желательно исключить продукты, повышающие выработку гистамина (клубника и цитрусовые, шоколад и кофе, арахис).
