Description

 Боль в глазу возникает при ожогах, травмах, воспалительных заболеваниях, аллергических реакциях, повышении внутриглазного давления (ВГД). Болезненные ощущения в глазах могут сопровождать неврологическую патологию: неврит, невралгию, мигрень. Для определения причины боли в глазу проводится биомикроскопия, бесконтактная тонометрия, офтальмоскопия, ультразвуковое исследование, рентгенография орбиты. Для устранения боли используются анальгетики. Этиотропное лечение зависит от основной патологии.

Reasons

Ожог глаза.

 Ожог, независимо от его вида (химический, термический), вызывает сильную боль, интенсивность которой со временем нарастает. Пациент не может открыть глаза самостоятельно. Сопутствующие проявления - светобоязнь, усиление слезотечения, отек век и хемоз конъюнктивы. При сильных ожогах гиперемия сменяется побелением тканей глаза.
 • Инородное тело. Они могут располагаться на поверхности глаза или иметь внутриглазную локализацию. В практике офтальмолога чаще встречаются инородные тела в конъюнктиве, веках и роговице. Расположение инородных частиц под верхним веком травмирует роговицу во время моргания, вызывая боль.
 • Эрозия роговицы. Появление эрозионных дефектов часто бывает вызвано микротравмами при макияже, продувкой веточкой. Симптомы особенно заметны при пробуждении и включают боль, покраснение, слезотечение, нечеткость зрения и светобоязнь.
 • Аденовирус. Для этого конъюнктивита характерны усиленный хемоз, гиперемия и боль. Течение болезни затягивается (3 недели и более). Часто патологический процесс распространяется на роговицу.

 • Биомикроскопия глаза. В первую очередь проводится детальный осмотр переднего отрезка глазного яблока с обязательной инверсией верхнего века. Затем его окрашивают флуоресцеином и исследуют с помощью кобальтового фильтра, благодаря чему можно сделать видимыми небольшие эрозионные дефекты.
 • Офтальмоскопия. Исследование глазного дна проводится после циклоплегии, если внутриглазное давление компенсируется. Офтальмолог оценивает прозрачность оптической опоры и состояние сетчатки вплоть до зубчатой ​​линии.
 • УЗИ глаза. Ультразвуковое исследование применяется при затруднениях в визуализации структур глазного яблока из-за миоза, отека роговицы, гифемы или гемофтальма. Преимущество этого метода - возможность идентифицировать рентгеновские отрицательные инородные тела.
 • Рентгенография. Его проводят при тяжелых травмах, чтобы исключить повреждение костных стенок орбиты. Специальный протез Комберга-Балтина позволяет определить локализацию внутриглазных рентгеноконтрастных инородных тел.

Treatment

 Лечебная тактика зависит от этиологии глазной боли. Обосновано указание на этиотропные и патогенные средства. Не следует отдавать предпочтение физиотерапевтическим и народным средствам для снятия боли. Консервативное лечение может включать:
 • Обезболивающие. Детоксифицирующие капли используются на этапе диагностики, а также перед проведением субконъюнктивальных инъекций. При сильном болевом синдроме показан прием пероральных анальгетиков, так как местные средства подавляют процесс регенерации.

 • Ремонт. Эти препараты помогают регенерировать роговицу. Стимуляторы восстановления тканей показаны при эрозиях, посттравматических дефектах и ​​послеоперационных ранах. Продолжительность использования варьируется от 10 дней до 1 месяца.
 • Антигистаминные препараты. При аллергическом конъюнктивите патогенетическая терапия сводится к назначению блокаторов гистамина. Однако следует определить тип аллергена и назначить этиотропную терапию аллергологом.
 Проникающие раны глаза требуют первичной хирургической обработки раны с ушиванием дефекта. При наличии инородного тела его микрохирургическое удаление показано на ранней стадии.
