Description

Агрессивность. Это качество личности, выражающееся в действиях и поступках, ориентированных на причинение вреда окружающим или самому себе. Проявляется враждебностью, нанесением физических травм, произнесением оскорблений. Агрессивные люди раздражительны, обидчивы, часто переживают гнев, злость, ярость. Диагностика выполняется клиническими методами, при помощи психодиагностических личностных опросников, проективных тестов. Для снижения агрессивности применяется психоанализ, когнитивно-поведенческая психотерапия, аутотренинг, медикаментозная коррекция.

Symptoms

С практической точки зрения агрессивность рассматривается как повышенная готовность человека к агрессивному поведению, которое не является врожденным, а приобретается в процессе социального взаимодействия. По качественным характеристикам различают агрессивность конструктивную и разрушительную. Первая реализуется в оборонительных действиях, нанесении непреднамеренного вреда, позволяет социально адаптироваться, преодолевать препятствия, проявлять лидерство. Вторая представлена неконструктивным агрессивными действиями, основанными на умысле причинения физического или психологического страдания другому.
По направленности агрессивность бывает ориентированной на окружающих и на себя. В соответствии с этим выделяют гетероагрессивные - оскорбления, побои, ругань и самоповреждающие или аутоагрессивные реакции. Крайним вариантом аутоагрессии являются попытки суицида. Наиболее очевидные враждебные действия - использование физической силы против другого лица. Прочие возможные варианты - неприязнь, ехидство, негативизм, раздражение, обидчивость, подозрительность, высказывание угроз. Агрессивность, направленная на себя, сопровождается чувством вины, самоуничижением.

Причины.

Становление агрессивности как черты личности - сложный процесс, в котором задействовано множество факторов. Враждебное поведение формируется постепенно под влиянием семьи, сверстников, средств массовой информации. Дети учатся агрессии путем наблюдения и апробирования: видят, как можно проявить ненависть и враждебность, оценивают, к чему это приводит, реализуют полученные знания на практике. Если опыт агрессивных поступков положительный, они закрепляются в характере. Причины агрессивности взрослого человека - социально-психологические факторы, психические болезни, состояния алкогольной и наркотической интоксикации.
• Ситуативные. Агрессивность провоцируется раздражающими внешними условиями и физиологическими состояниями. Так, более вспыльчивыми люди становятся при перегреве, нахождении в душном помещении, длительном воздействии шумов, вибраций. Самоконтроль ослабевает в ситуации стресса, при усталости, болевых ощущениях, половом возбуждении.
• Поведенческие. К данной группе факторов относится любое поведение, которое расценивается окружающими как неприемлемое, вызывает ответную защитную реакцию. Например, агрессивность ребенка бывает связана с бесцельным времяпрепровождением, прогулами уроков, употреблением алкоголя - поступками, вызывающими осуждение родителей, педагогов.
• Личностные. Агрессивными чаще бывают люди с сангвиническим и холерическим темпераментом, с возбудимыми и асоциальными чертами характера. Враждебное поведение реализуется на основе эмоциональной неустойчивости, раздражительности, депрессивности. Другие личностные причины - неадекватный уровень самооценки, готовность к риску, завистливость.
• Микросоциальные. Микросоциальной средой является семья, школьный или трудовой коллектив. К демонстрации агрессии склонны люди, которые часто наблюдают насилие или испытывают его на себе, переживают унижения, оскорбления. Поэтому высокий уровень враждебности определяется у подростков из неблагополучных семей, у людей, работающих в силовых структурах.
• Макросоциальные. Как причины агрессивности могут рассматриваться общественные, политические и культурные факторы. Враждебная настроенность формируется при пропаганде негативных событий СМИ, демонстрации ненормативного поведения значимыми людьми (руководителем страны, министрами), возведении в культ насилия. Провоцирующим фактором часто становится низкое социально-экономическое положение, зависимость от материальной помощи государства.

Diagnostics

Диагностика агрессивности и ее причин проводится врачом-психиатром, психотерапевтом, клиническим психологом. Особый интерес представляют случаи определения склонности к агрессии при выполнении экспертиз, когда у пациента может присутствовать стремление скрыть нежелательные качества. В таких ситуациях помимо стандартных диагностических процедур используются экспериментальные методы, в ходе которых осуществляются воздействия, провоцирующие враждебность. В рамках базового исследования применяются следующие процедуры:
• Опрос. Во время беседы врач выясняет причины агрессивности, ее продолжительность, выраженность, обоснованность. Пациенты не всегда критичны к изменениям своего поведения, поэтому для получения более объективной информации проводится опрос родственников (сопровождающих лиц), запрашиваются характеристики из школы, с места работы.
• Наблюдение. Склонность к агрессивным реакциям проявляется во время врачебной консультации: больные раздражительны, вспыльчивы, грубы. На неприятные вопросы отвечают с руганью, легко переходят к обвинениям, ссорам, скандалам. Более полное представление о поведении пациентов дает наблюдение в условиях стационара. Как правило, они проявляют вербальную и физическую агрессию, становятся зачинщиками ссор с медицинским персоналом, другими больными.
• Психодиагностическое тестирование. Личностные опросники позволяют количественно оценить выраженность агрессивности, определить ее положение в структуре характера, сочетание с другими качествами, например, импульсивностью. Используется тест Басса-Дарки (Диагностика состояния агрессии), тест А. Ассингера (оценка агрессивности в отношениях). Для выявления скрываемой агрессии эффективны проективные методики: тест руки, тест Розенцвейга.

Treatment

Подбор медикаментозного лечения необходим при тяжелых формах агрессивности, когда поведение пациента представляет угрозу для него самого или окружающих. Прием лекарств позволяет достичь компенсации общего состояния, снизить количество агрессивных поведенческих актов, предупредить их развитие в будущем. Распространено комбинированное использование быстродействующих бензодиазепинов с нейролептиками. Фармакологическое воздействие объясняется способностью препаратов влиять на уровень и соотношение нейромедиаторов. Помимо вышеуказанных лекарств могут быть назначены атипичные нейролептики, соли лития, бета-адреноблокаторы, СИОЗС.

Лечение.

Агрессивность не должна рассматриваться как неизбежная форма реагирования человека на конфликты, жизненные трудности и неудобства. При своевременной правильной работе над своими личностными качествами можно научиться контролировать проявления злости и враждебности, предупреждая развитие их патологических форм. Симптоматическая медико-психологическая помощь заключается в проведении психотерапевтических сеансов, групповых тренингов, медикаментозной коррекции.
