Pain in the heart

Description

 Боль в сердце возникает как по сердечным, так и не по сердечным причинам. Наиболее частые патологии, сопровождающиеся болевым синдромом: стенокардия, артериальная гипертензия, аритмии. Симптом типичен для миокардита и перикардита, ревматизма, кардиомиопатии.  Сердечный невроз - основная причина сердечных болей. Симптомы чаще встречаются у эмоционально нестабильных людей молодого и среднего возраста, подростков и беременных женщин. Болезненные ощущения характерны для кардионевроза, сила и характер которого различны. Больные жалуются на давящие, жгучие, колющие боли. Жалобы проявляются в основном стрессом и волнением.
 Специфический признак кардионевроза - постоянный страх серьезного порока сердца, инфаркта миокарда. Пациент начинает прислушиваться к своим ощущениям и трактует даже малейшую боль в сердце как признак смертельной болезни. Помимо боли наблюдается повышенное потоотделение, ощущение жара или озноба и дрожание пальцев. После нормализации психоэмоционального состояния симптомы бесследно исчезают.

 Первичный прием к кардиологу начинается с анамнеза, внешнего осмотра и физикального обследования сердечно-сосудистой системы. При аускультации сердца слышна глухота тонов, функциональные или органические шумы. Артериальное давление измеряется на руках и ногах и при необходимости назначается СМАД. Для установления этиологии болей в области сердца проводится ряд лабораторных и инструментальных методик:
 • ЭКГ. На кардиограмме часто наблюдаются признаки гипертрофии левого желудочка, нарушения ритма и проводимости, симптомы ишемии сердечной мышцы. Повышение сегмента ST в сочетании с инверсией зубца T является патогномоничным для ИМ. Для уточнения диагноза проводятся функциональные нагрузочные пробы с физической нагрузкой.
 • ЭхоКГ. На основании данных УЗИ оценивается анатомическое строение сердца, работа клапанов и толщина миокарда. Эхокардиография измеряет фракцию выброса левого желудочка. Для исследования кровотока в коронарных артериях и крупных сосудах проводится ультразвуковая допплерография.
 • Радиографические методы. Для визуализации размеров и контуров сердца, расчета кардиоторакального индекса рекомендуется рентгенография OGC. Детальное обследование коронарных артерий, легочных сосудов и камер сердца требует компьютерной томографии сердца.
 • Ишемическая ангиография. Инвазивная методика показана при вероятном развитии инфаркта миокарда, стенозе коронарных артерий. В сосудистое русло вводится контрастное вещество, которое позволяет визуализировать внутреннюю поверхность артерий сердца, выявить тромбы и атеросклеротические бляшки.
 • Лабораторные методы. Повышение холестерина и холестерина ЛПНП в биохимическом анализе крови свидетельствует об атеросклерозе. Для подтверждения инфаркта миокарда во время приступа стенокардии определяется уровень миоглобина, тропонина и сердечных ферментов. Чтобы исключить ревматизм, измерьте показатели острой фазы, антистрептолизин-О.
 В связи с частым возникновением некардиальных причин сердечной боли обычно необходима консультация других специалистов. При возможном ГЭРБ, EFGDS рекомендуется рН-метр. Для оценки состояния ребер, грудного отдела позвоночника делают рентген в двух проекциях, КТ, МРТ. При подозрении на кардионевроз проводятся анализы для оценки функционального состояния вегетативной нервной системы.

Treatment

 После проверки основного диагноза проводится комплексное этиопатогенетическое лечение. Если выявляются проблемы с пищеварительной системой, пациента направляют к гастроэнтерологу. Лечением остеохондроза занимается невролог, вертебролог. При болях в сердце, связанных с сердечно-сосудистой патологией, назначают:
 • Антигипертензивные препараты. Препараты снижают преднагрузку и постнагрузку в сердце, за счет чего миокард начинает нормально сокращаться. Это помогает устранить болезненные ощущения, а также резко снижает вероятность гипертонических кризов, сердечных приступов и инсультов у гипертоников.
 • Антиаритмические препараты. Препараты стабилизируют частоту сердечных сокращений и ритм, координируют сократительную способность предсердий и желудочков. После восстановления сердечного ритма исчезают боли и дискомфорт, улучшается общее самочувствие.
 • Антиангинальные препараты. Лекарства рекомендуются при ишемической болезни сердца. Они улучшают кровообращение и трофику сердечной мышцы, уменьшают преднагрузку. При этом восстанавливается обратимое повреждение миокардиальных волокон и уменьшаются типичные болевые приступы.
 • Тромболитики. Препараты назначают в первые часы после диагностики инфаркта миокарда. Они разрушают тромбы в коронарной артерии и восстанавливают нормальный кровоток. С его первоначальным применением можно максимально отграничить зону некроза.
 • Противовоспалительные препараты. НПВП используются при воспалении. Препараты снижают выработку цитокинов, вызывающих сердечную боль. При ревматических травмах следует назначать глюкокортикостероиды.
 Лечение кардионевроза начинается с выбора немедикаментозных методов. Пациентам рекомендуется нормализовать свой распорядок дня, увеличить время сна и отдыха и по возможности избегать стрессовых ситуаций. При серьезных психоэмоциональных проблемах эффективна индивидуальная и групповая психотерапия. Травяные успокаивающие средства используются для нормализации работы центральной нервной системы.
 Вмешательство кардиохирургов требуется, если консервативная терапия инфаркта миокарда неэффективна или невозможна. Для восстановления коронарного кровотока применяют транслюминальную баллонную ангиопластику (ТЛБАП), чрескожную ангиопластику с установкой стента на месте стеноза артерии. Реже выполняется аортокоронарное шунтирование (АКШ).
 При тяжелых аритмиях необходимо хирургическое лечение. В большинстве случаев для поддержания правильной частоты пульса имплантируется кардиостимулятор. При злокачественных тахиаритмиях показана установка дефибриллятора. Для устранения мерцательной аритмии, экстрасистолии применяется методика радиочастотной абляции.

 В большинстве случаев сердечная боль возникает при эмоциональном и физическом стрессе, поэтому человеку приходится сесть или лечь, чтобы попытаться успокоиться. Необходимо расстегнуть воротник, снять тесную одежду, обеспечить свежий воздух. В случае перебоев в работе сердца желательно сделать глубокий вдох и заняться спортом, надавливая пальцами на закрытые глазные яблоки. Сосудорасширяющие средства используются для купирования боли при стенокардии.
