Dangerous interactions
Inhibitors of cytochrome P450 isoenzyme 3A4 (ketoconazole, itraconazole, erythromycin, miconazole, clarithromycin, fluconazole, troleandomycin) increase plasma concentrations and increase the risk of fatal arrhythmias. Cisapride (A03FA02) + Fluconazole => Increases the concentration of cisapride in blood plasma, and increases the risk of fatal arrhythmias. Against the background of fluconazole, the biotransformation of cisapride slows down, the risk of cardiovascular disorders increases, in particular fatal arrhythmias (paroxysmal ventricular tachycardia). Concomitant administration of cisapride and fluconazole is contraindicated (see 'Contraindications'). Concomitant use of cisapride and fluconazole is contraindicated. -
Negative interactions
The use of fluconazole simultaneously with drugs metabolized with the participation of the cytochrome P450 enzyme system (in pm, cisapride, astemizole, quinidine) may lead to an increase in serum levels of these drugs (in the absence of accurate information, caution should be exercised and the condition of patients carefully monitored). Controlled studies have shown that the use of fluconazole at a dose of 200 mg once a day and cisapride at a dose of 20 mg 4 times a day for 5 days leads to a marked increase in plasma concentrations of cisapride and an increase in the QT interval on the ECG. With the simultaneous use of fluconazole and cisapride, undesirable reactions from the heart are possible, in pm, ventricular tachysystolic arrhythmia of the 'pirouette' type (torsade de pointes). The use of fluconazole at a dose of 200 mg once a day and cisapride at a dose of 20 mg 4 times a day leads to a marked increase in plasma concentrations of cisapride and an increase in the QT interval on the ECG. Like other antifungal agents (azole derivatives), fluconazole, when used concomitantly with warfarin, increases prothrombin time (by 12%), which may lead to the development of bleeding (hematomas, nosebleeds and gastrointestinal tract, hematuria, melena). -

Decoding the colors of interactions and contraindications
Dangerous | — | a pronounced negative interaction or contraindication. |
Negative | — | negative interaction or side effect that may reduce effectiveness. |
Positive | — | the interaction can SOMETIMES be used as a positive (often a dose adjustment is needed), or it is an indication of the drug. |
No | — | the drugs do NOT interact, which is separately indicated in the instructions. |
Unclear | — | the system failed to pre-assess the danger. |

Additional information
- Kiberis checks interactions and evaluates drug compatibility online right in the instructions thanks to the latest artificial intelligence technologies. The accuracy of finding is more than 95%, the accuracy of the hazard assessment is more than 80%. The online medical service takes into account all the drug groups of the selected drugs and all their components. And since the database contains 25,000 drugs with detailed instructions, not every pharmacologist can compete with our artificial intelligence. List of popular interactions.
