Description

 Артроз - это преждевременное изнашивание внутрисуставного хряща. Одна из причин, приводящих к заболеванию, старение хондроцитов, клеток хрящевой ткани. Постепенно может уменьшиться общее количество хряща в суставе, особенно ощутимое с приближением пожилого возраста, а также после травм, посттравматических воспалений, профессиональных постоянных нагрузок на определенные суставы (например, у футболистов и работников сельского хозяйства). Наиболее часто при остеоартрозе поражаются коленный (гонартроз) и тазобедренный (коксартроз) суставы. Одним из наиболее ранних симптомов артроза является боль в коленных суставах. В начале заболевания в покое она практически отсутствует, но появляется при нагрузке на сустав.
 Артроз первичный (генуинный) - артроз, начинающийся без заметной причины и поражающий неизмененные суставные хрящи во многих суставах одновременно; наблюдается чаще у лиц старше 40 лет.
 Артроз вторичный - является исходом заболеваний или повреждений суставов.
 Артроз генерализованный (полиартикулярный, полиартроз) - артроз, характеризующийся множественным поражением суставов.
 Артроз деформирующий - артроз, характеризующийся выраженными деструктивными и гиперпластическими изменениями суставных концов костей, проявляющийся сильными болями, значительной дефигурацией суставов и прогрессирующим нарушением функции; чаще поражаются тазобедренные и коленные суставы.
 Артроз унковертебральный - деформирующий артроз суставов, дополнительно формирующихся между отростками на заднелатеральных поверхностях II - II шейных позвонков; проявляется признаками шейных и плечевых невритов и другими неврологическими симптомами.
Arthrosis

Symptoms

 Вначале болезненные изменения появляются во внутренней (т. н. синовиальной) оболочке суставной сумки, затем они захватывают хрящ, покрывающий суставные поверхности сочленяющихся костей; хрящ постепенно разрушается, обнажая кость; костная ткань местами разрежается, местами уплотняется, образуются костные шипообразные выросты - развивается картина деформирующего А. Чаще А. развивается в тазобедренном, коленном и первом плюсне-фаланговом суставах. Обычно болеют люди среднего и пожилого возраста. Заболевание проявляется болями, которые появляются исподволь, возникают периодически, обостряются после резких физических нагрузок или, наоборот, после длительного состояния покоя. Вследствие болей ограничивается подвижность в суставе. Возникают воспаления тканей, окружающих сустав, и нервных стволов, функция сустава страдает и вследствие защитного напряжения мышц.
 Ассоциированные симптомы: Асимметрия. Асимметрия суставов. Боль в запястье. Боль в кисти. Боль в ключице. Боль в локтевом суставе. Боль в пальцах рук. Боль в пятке. Деформация. Деформация пальцев рук. Ломота в суставах. Ломота в теле. Ограничение амплитуды движений. Опухание суставов. Отеки ног. Покалывание в области сердца. Слабость мышц (парез). Судороги. Судороги в ногах. Тяжесть в руках. Хромота.

Reasons

 Может возникнуть в результате интоксикаций, инфекционных заболеваний (например, тиф, сифилис и ), травмы сустава (перелом суставных концов костей, повреждение суставного хряща), а также при значительной функциональной перегрузке сустава (например, у артистов балета, грузчиков и ). Известное значение имеет простуда (например, А. у рабочих горячих цехов). В основе заболевания лежит нарушение питания суставных (эпифизарных) концов костей. Вследствие изменения проходимости или повреждения питающих кость сосудов возникают асептические некрозы, усугубляющие поражения в суставе. Т. о., заболевание носит прогрессирующий характер.

Treatment

 Лечение - амбулаторное и в санаторно-курортных условиях (Цхалтубо, Евпатория). Назначают обезболивающие средства, гормональные препараты (адренокортикотропного ряда), физиотерапию (тепловые процедуры, ультразвук), лечебную гимнастику, массаж. В тяжёлых случаях - хирургическая операция (артродез, артропластика).
