Description

 Гидроксиапатитная артропатия. Воспалительное поражение одного или нескольких суставов, связанное с отложением в его периартикулярных тканях гидроксиапатита кальция. Клинически проявляется ограничением движений в суставе, локальной болезненностью и отечностью тканей в местах прикрепления к суставу сухожилий. Наиболее часто отмечается поражение плечевого сустава. Диагностика гидроксиапатитной артропатии основана на клинической картине, данных рентгенографического исследования суставов, определении уровня кальция в крови и моче, исследовании синовиальной жидкости. Лечение проводится нестероидными противовоспалительными препаратами и физиопроцедурами.

Additional facts

 Гидроксиапатитная артропатия встречается в основном у пациентов после 40 лет. По некоторым статистическим данным средний возраст заболевших составляет 44 года. Среди женщин случаи заболевания гидроксиапатитной артропатией отмечаются чаще, чем среди мужчин. В 75% случаев гидроксиапатитной артропатии наблюдается поражение плечевого сустава. Далее по частоте встречаемости следует поражение суставов кисти, лучезапястного сустава, позвоночника и коленных суставов. В отдельных случаях возможно воспаление тазобедренного или голеностопного сустава, еще реже - мелких суставов стопы.

Reasons

 Причины развития первичной гидроксиапатитной артропатии пока неизвестны. Предполагают, что отложение кальция в периартикулярных тканях может быть обусловлено нарушением обмена веществ, воздействием профессиональных вредностей или травмирующих факторов. Гиперпаратиреоз, гемохроматоз, гемодиализ, длительный прием препаратов витамина D могут стать причиной вторичной гидроксиапатитной артропатии. Семейные случаи гидроксиапатитной артропатии, частое выявление у заболевших HLA A2 и HLA в35 указывают на наличие генетической предрасположенности.
 При гидроксиапатитной артропатии происходит отложение кристаллов гидроксиапатита кальция в окружающих сустав сухожилиях и синовиальной оболочке сустава, что приводит к возникновению реактивного воспалительного процесса. Из синовиальной оболочки кристаллы гидроксиапатита могут проникать в находящуюся в суставе синовиальную жидкость, но там они быстро рассасываются. Поэтому при проведении исследований синовиальной жидкости в ней выявляется повышенное содержание кальция, а сами кристаллы обнаруживаются довольно редко.

Classification

 Современная ревматология выделяет 4 клинических варианта гидроксиапатитной артропатии. Моноартрит - этот вариант гидроксиапатитной артропатии составляет 65% от всех случаев заболевания. Характеризуется поражением одного, максимум двух, суставов с выраженными симптомами артрита. Поражение позвоночника - второй по частоте встречаемости вариант гидроксиапатитной артропатии. На его долю приходится около 35% случаев заболевания. Проявляется острыми приступами радикулита в различных отделах позвоночника, сопровождающимися повышением температуры тела.
 Острый псевдополиартрит - представляет собой острое множественное поражение суставов, сопровождающееся подъемом температуры тела и значительным ускорением СОЭ. Его клиника напоминает течение ревматоидного артрита или полиартрита инфекционной этиологии. Однако тщательный осмотр пораженных суставов позволяет выявить периартикулярный характер воспалительных изменений.
 Хронический псевдополиартрит - отличается длительным течением с постоянными артралгиями на фоне незначительно выраженных воспалительных изменений в области многих суставов. При этом варианте гидроксиапатитной артропатии нередко возникают острые приступы с развитием синовиита, не приводящие к деформации сустава.

Symptoms

 Клинические проявления гидроксиапатитной артропатии характеризуются периодическими приступами острого или подострого артрита одного или нескольких суставов и зависят от локализации поражения. Чаще всего вне обострения гидроксиапатитной артропатии все клинические симптомы заболевания проходят до следующей атаки. В отдельных случаях наблюдается хроническое течение с постоянным присутствием малоинтенсивных суставных болей в период между атаками.
 Поражение сухожилий лопатки и плеча проявляется картиной плече-лопаточного периартроза. Характерна болезненность и отечность в области крепления сухожилия к суставу, ограничение движений в суставе, в наибольшей степени - нарушение отведения. Подобная картина наблюдается в тазобедренном суставе при поражении крепящихся к нему мышц.
 Гидроксиапатитная артропатия в области локтевого сустава обычно характеризуется картиной эпикондилита с ограничением движений и болями в области прикрепления к суставу трицепса, общего разгибателя и сгибателя кисти. Гидроксиапатитная артропатия кисти и пальцев проявляется усиливающимися по ночам тупыми постоянными болями, утренней скованностью, незначительной припухлостью пораженных суставов. Клиническая картина может развиваться по типу теносиновита сгибателей, реактивного артрита, синдрома карпального канала. Отличительной особенностью является отсутствие деформаций в суставах кисти.
 Ассоциированные симптомы: Асимметрия суставов. Боль в запястье. Боль в кисти. Лейкоцитоз. Ломота в суставах. Опухание суставов.

Diagnostics

 В остром периоде гидроксиапатитной артропатии в клиническом анализе крови наблюдается ускорение СОЭ, лейкоцитоз. Для выявления причины возникновения гидроксиапатитной артропатии ревматолог назначает определение уровня кальция в крови и моче, однако этот показатель может быть не увеличен.
 Типичным для гидроксиапатитной артропатии является выявление множественных кальцификатов в периартикулярных тканях при проведении рентгенографии позвоночника и суставов. При этом не наблюдается корреляция между количеством кальцификатов и выраженностью клинической картины. Выраженная клиника заболевания может сопровождаться отсутствием очагов кальцификации и, наоборот, выявленная рентгенологически кальцификация может не сопровождаться никакими проявлениями заболевания. Типичным является исчезновение кальцификатов после перенесенного приступа гидроксиапатитной артропатии.
 Забор синовиальной жидкости производят путем пункции сустава. При гидроксиапатитной артропатии кристаллы гидроксиапатита редко выявляются в синовиальной жидкости, обычно отмечается лишь повышенное содержание кальция.

Differential diagnosis

 Дифференциальный диагноз гидроксиапатитной артропатии проводят с ревматоидным артритом, подагрой, остеохондрозом позвоночника, деформирующим спондилезом, тендинитами, интермиттирующим гидрартрозом, палиндромным ревматизмом.

Treatment

 Острые приступы гидроксиапатитной артропатии эффективно купируются приемом нестероидных противовоспалительных (диклофенак, индометацин, ибупрофен). Выраженная симптоматика может быть показанием к внутрисуставному введению гидрокортизона, триамцинолона. Возможно обкалывание этими препаратами мягких тканей вокруг сустава. В лечении хронической формы гидроксиапатитной артропатии широко используются физиотерапевтические методы: ультрафонофорез гидрокортизона, электрофорез йода, грязелечение, парафиновые аппликации, сероводородные и радоновые ванны.
