Mental

 Makes mistakes in writing and speaking; puts right for left and vice versa, and I for you; (the above mentioned sides, right and left, I carefully ascertained to be correct), (sixth day).

Head, face, and ears

 Vertigo. Light headed; felt as if head were going round, and as if he would fall to left side; objects see to run into one another; becoming confounded had to stand still to recover him self; this lasted two or three minutes, at 3. 30 P. M. (fourth day). Sensation as if he would fall to the left; in fact he staggered a little to the left, when walking down a street, in the evening (first day),. Staggering to the left twice when walking in room, directly after rising fro bed, at 4. 20 p.m (second day); slight staggering to the left, when rising from kneeling (after eleven hours, (third day). Forehead. Sharp pain all over forehead for thirty minutes (after four hours, first day). Sensation on right side of top of forehead, as if it were being cut open, about an inch from behind forwards, for 3 minutes, at 7. 30 P. M. (twenty-first day). Momentary throbbing in right frontal eminence, immediately afterwards a momentary cessation, as if a heavy weight were being pulled over it, at 8. 5 P. M. (seventeenth day). Temples. Pricking as with a needle in right temporal muscle, for one minute, at 5. 15 P. M. (eighth day). Vertex. Feeling of a piece of ice on center of vertex (after fifteen minutes). Painful boring in right side of vertex, with slight pressure there, for ten minutes, at 7. 15 P. M. (fourth day). Rheumatic drawing pain in right side of vertex, aggravated by bending head down to read, for ten minutes, at 11. 5 P. M. (fourth day). Great weight in vertex when the pain in stomach occurs; cannot sit to his business (office work), in the morning; pain at vertex sometimes reaches down to over eyes. Sharp shoot into brain on left side of vertex, leaving a little feeling of soreness there, but not sore to touch, for one minute, at 10. 10 P. M. (sixth day).
 Eyes.
 Slight twitching in external corner of left supraorbital ridge, for half a minute, at 4. 30 P. M. (seventeenth day). Twitching in right upper eyelid at intervals during afternoon, from 1. 30 P. M. till 7 P. M. (twenty first day). Pricking pain in right upper eyelid, with fullness of forehead, mostly on right side, for half an hour, directly after dinner (1 P. M)., (second day). A feeling at right outer canthus as if it had been bruised; it is felt on working or rubbing that part; the pain is in the integuments, not in the eyeball (thirty-first and thirty-second days); a small stye on inner surface of right upper lid near external canthus; broke the stye, after which the former pain gradually subsided (thirty-third day). Eyes very sensitive to sunlight, on going outdoors at 10 p.m; he had to close them and walk on the shady, side of the street (ninth day).
 Ears.
 External. сutting pain externally behind right ear (in mastoid process), going a little way down neck, for two minutes, at 10. 15 P. M. (eighth day). A rather severe, continuous, lancinating pain behind right ear, on a level with meatus and about one inch distant, for two or three minutes, at 10. 30 P. M. (eighth day). Internal. Stitch in right internal ear, fro jaw along Eustachian tube, when chewing on that side, for one minute, at 5. 20 P. M. (seventeenth day). сontinued stitch from left ear to left tonsil along Eustachian tube, for ten minutes, at 10 p.m (sixth day). Sensation in left internal ear, as if it opened and shut twice, at 6. 45 P. M. (fourth day). Transitory sensation of fluttering in right internal ear, as if opening and shutting; this was shortly followed by a similar sensation in left ear, and afterwards in both ears at same time; relieved by putting a finger in each ear; it disappeared in right before occurring in left, and in left before occurring in both (within an hour).
 Face.
 Right side of the face swollen in morning, increasing during day; in afternoon cold air out of doors increased the pain, which was relieved by slight pressure of hand; pain worse on laughing; in evening it became very painful (sixth day); increased; slight pressure of hand and cold water relieved the pain (seventh day). Slight throbbing in left cheek-bone for one minute, at 2. 25 P. M. (fourth day). Lips dry, on waking (sixteenth day). Soreness on pressure under jaw on right side, from angle half way towards symphysis (eighth day).

Mouth and throat

 Teeth. Sensation of sharp coldness in first left upper bicuspid, increased by drawing in the air, during the afternoon (ninth day). Dull aching in first lower bicuspid and first upper molar, both of left side, slightly relieved by clenching teeth, at 2. 45 P. M., lasting all day (molar is stopped, all other teeth which have been affected are sound), (sixteenth day). Stitches in a decayed second left upper molar occasionally, especially when touching tooth with cheek or tongue, or biting on tooth (third day); stitches in same tooth, with dull pressing pain during the interval between the stitches; the pains were worse when lying on the painful side; when lying on painless side they went off and he felt asleep; when he awoke it was always on the painful side (third night); the pains continued at intervals, not affected by hot or cold drinks; in evening on entering a warm room the pain increased to a throbbing, with heat of face, especially left side; on going into a cooler atmosphere the pain ceased (fourth day). Shooting in first left upper molar tooth, with sensation of coldness in it, for three minutes, at 4. 20 P. M. (sixth day). Pain in second bicuspid and first molar of right upper jaw, both of which are stopped, at 11 p.m; towards evening a small swelling on outer side of the corresponding gums (fifth day). Gums. After midnight, gumboil burst with great relief to pain, and towards evening the swelling of the face nearly disappeared (ninth day). Mouth. сlammy feeling in mouth, on waking (third day). Great burning feeling in mouth and fauces, extending into the stomach, not relieved by cold water or anything else. Taste. Disagreeable taste in mouth, which lasted some time, on waking (sixteenth day). Woke at 7. 30 p.m, with a bloody taste in mouth, which went off after cleaning teeth (twenty-third day).

Gastrointestinal tract

 Burning, as fro vitriol, in the stomach.
 Abdomen.
 Shifting of flatulence in left side of abdomen, with audible rumbling, when sitting still, going off after standing up for fifteen minutes, at 4. 45 P. M. (third day). Slight pinching and gripping in bowels, as if diarrhoea were coming on, causing, however, only an ordinary stool but with passage of much flatus; all being relieved by stool, at 6 P. M.; much shifting of flatus, with slight pinching pains in bowels, after getting into bed; both symptoms relieved by lying on back for a short time, at 11 P. M. (eleventh day). Pain in right groin as if inguinal hernia would protrude, for ten minutes, when walking, at 10 p.m (seventeenth day).
 Rectum.
 Desire to evacuate contents of bowels, whenever he thought about it, lasting a few days (third day).

Urogenital system

 Severe burning in forepart of the urethra during micturition, and for two minutes afterwards, at 11 P. M. (second day). Urine last night and today much increased, passed much at a time, but not more often than usual; in afternoon urine pale (seventh day); high-colored, in afternoon after standing, considerably loaded with lithates (it was tasted), (eighth day); high-colored, in the morning, with a thick deposit, in the evening (ninth day); slight deposit in evening and night urine (tenth and twelfth day); rather high colored, no deposit (fourteenth day); rather more high-colored, very slight turbidity (fifteenth day); somewhat high-colored (sixteenth day); slightly so (seventeenth day); slightly high-colored, but quite clear (twentieth to twenty- seventh days); occasionally high-colored for six weeks after wards.

Chest organs

 Tickling at right side of larynx, as if fine dust were collected there, causing a dry cough for three minutes, at 7. 25 P. M. (thirteenth day).
 Chest.
 Sharp sticking pain in right lung, as if knives were being thrust into it fro the front, for five minutes, at 11. 15 p.m (fifteenth day). Sharp stitch through upper half of left lung, from before backwards, at 9. 30 p.m (third day). A painful bruised and empty sensation in upper lobe of left lung, extending into left axillae, for three minutes, only when expiring, when lying on left side in bed, at 11 P. M. (twenty-third day). Sides. Sharp pressure on right side of chest, as if the chest were pressed on right lung, for one minute, at 8. 40 p.m (third day). Transitory stitch in left side, under last rib, arresting breathing, at 9. 30 P. M. (second day). A sudden stitch under last rib of left side, whilst riding in a carriage, at 11. 30 p.m (eighteenth day). Sharp shooting in right side of chest, worse on deep inspiration, for two minutes, at 10. 45 P. M. (eleventh day). A sharp shoot up right side of body fro right hypochondrium to middle of right side, causing him to start, at 12. 40 P. M. (fifteenth day).  Appetite poor.
 Neck. Jactitation of muscles on left side of neck, drawing head suddenly from right side (on which he was lying in bed), towards left, leaving a sensation as if right cervical muscles had been sprained, at 11. 15 P. M. (sixth day). Occasional pain in left side of neck when moving neck, in afternoon (thirteenth day). Sensation in left cervical muscles, as if stiff from a cold or sitting in a draft, for ten minutes, between 6 and 7 P. M. (second day). Sensation as if bruised in left posterior cervical muscles, for one minute, at 6. 45 P. M. (sixteenth day). Painful throbbing on left posterior cervical muscles, for five minutes, at 10. 40 p.m (fourth day). Dorsal. When sitting bent over a table in the act of writing, very severe aching pain between shoulders, with feeling of tightness there, lasting fifteen minutes, going off gradually by sitting straight up, at 11 p.m (seventeenth day). Was awoke by a violent aching pain below right scapula, as if he had previously received a blow there; the pain extended forwards into the right side of chest, causing a sensation as if the right lung was pressed between the back and chest, with momentary oppression of breathing of right lung; the pain was relieved by lying on the left side and back, aggravated by lying on right; it lasted about ten minutes, in the early morning, before daybreak (second day). When washing face with right hand, sudden pain about inner border or left scapula as if sprained, leaving a dull aching pain there; the pain was worse in putting right arm round to back in order to touch the spot; slightly so with left arm; worse on rubbing face with right hand; exercising with left arm; does not affect it; the farther I put right arm around to back the more severe is the pain; when in the stooping posture it is much better or altogether gone; worse immediately after rising fro stooping; not influenced by expiration or inspiration; putting left arm in front of chest does not affect it; right arm very slightly so; hitting straight up with right arm and bringing it straight down again somewhat aggravates the pain, but not with the left; the pain feels as if it would prevent inspiration, just as a heavy weight would, but in reality respiration is quite easy; pain is worse on bending right shoulder round to front as much as possible, with left not so much, at 8. 25 p.m; the pain is not noticed when at rest; aggravations less severe, at 8. 50 p.m (seventh day); still remaining at 10. 45 P. M., in putting right hand back to scapula, going off in course of day (eighth and ninth days); when using right arm while dressing, sudden pain as if sprained at inner border of left scapula (just as on previous occasion);worse on putting right arm round to back so as to touch left scapula with right hand; it left a dull aching pain there worse as before; not worse on touching right scapula with left hand; slightly worse by hitting up perpendicularly with either hand, especially right; also worse on brushing hair, especially with right hand, in the morning; pain only felt when putting right hand to back, as before, and there it was less, at 5 P. M. (thirty-fourth day); only slight pain as yesterday afternoon (thirty-fifth day). Lumbar. Weak feeling across loins, as if tired, when sitting, with slight aching in region of right kidney, for ten minutes, at 10 P. M. (sixth day). Slight pain in left lumbar region, for a few seconds, at 10. 10 P. M.; had it before during the evening, lasting a longer time; momentary pain again at 10. 30 P. M. (first day). Stitch in left lumber region, during the act of stopping, at 9. 30 p.m; pain not felt at rest, at 10 p.m; not nearly so bad on touching back with right hand (seventh day).

Cardiovascular system

 Sharp shooting into heart, from before backwards for one minute, at 2. 45 P. M. (eleventh day). Pulse very soft, more compressible than usual, in the evening (eighth day).

Limbs and spine

 Superior Extremities.
 Sharp pain, commencing at posterior part of left shoulder-joint, quickly shooting down ulnar side of arm to wrist-joint, continuing all along the arm for one or two minutes, then suddenly disappearing; the place where it commenced was tender to pressure during the pain, at 5. 25 P. M. (third day). Sensation as if a knife were being thrust into tip of right ring finger, for a few seconds, at 7. 10 P. M. (thirteenth day). Violent throbbing along bones of right thumb, for two minutes, at 1 P. M. (twenty-first day). Sensation in right little finger, as if he had received a blow on it, making first phalanx feel stiff and the whole finger bruised, worse when trying to stretch the finger out straight at 10 o’ clock, lasting all day (twelfth day); same sensation on waking, they went off at 2 P. M. (thirteenth day).
 Inferior Extremities.
 Hip. Right hip-joint dislocated, causing him to limp, from noon to 2 P. M. (second day). Thigh. Twitching in right glutei muscles, when sitting, for two minutes at 9. 5 p.m; for one minute, when sitting at dinner, at 8 P. M.; while sitting on edge of chair at 9. 20 P. M. (twelfth day). сutting pain in muscles of right leg just above knee, on inside, about the insertion of vastus internus and sartorius, for four minutes, at 6. 20 P. M. (thirteenth day). Sudden stinging in a spot in outer side of left thigh for a moment, at 4. 10 P. M. (seventh day). Leg. Jactitation of muscles of left lower leg, drawing entire leg up towards body, at 11. 25 P. M. (sixth day). Foot. Aching drawing on under side of sole of left foot, extending half way to corresponding side of left calf, increased by trying to cross left leg over right, at 8. 20 p.m (tenth day). Sticking pain in sole of right foot, going through to instep, for about five minutes; it occurred when sitting with foot at rest; worse by pressing on ground with toes by means of action of calf muscles at 10. 30 P. M. (second day). Lancinating pain in hollow of right sole, leaving a sensation of heat behind for one or two minutes, at 7 P. M. (thirteenth day). Toes. Transitory twitch in second left toe, causing the whole left leg to jerk, at 10. 45 P. M. (sixth day),.

Skin

 Skin.
 An itching, raised blotch on right side of forehead, increasing in size when rubbed, which relieved the itching; after rising from bed, at 5. 45 p.m; nearly gone by 5 P. M. (seventeenth day). Pimple began to form on middle of chin, at 10 P. M.; it became a boil and was very painful, especially when moving the jaws (eighteenth day); punctured the boil and squeezed out pus (nineteenth day); still some remains of boil (twentieth day). On inner side of upper thigh (left) rather posteriorly, is a rather large red pimple, with some redness around it, tender to the touch and painful to the friction of the clothing while walking (thirteenth day); Pimple has become a pustule, which I opened and squeezed out pus (fifteenth day). A number of small red, circular, not elevated spots, came out round lower part of left leg, one being close to ankle, at 12. 30 P. M.; they itched very much, and after rubbing became burning, lasting about three quarters of an hour, and leaving a scab after scratching (fourteenth day). One of the pimples on leg, which was scratched to bleeding, is still to be seen as a red, elevated spot, without itching, at 5 P. M. (seventeenth day).

Sleep

 Not much sleep, restless towards morning (sixth night). Woke on back, with clammy feeling in mouth and slight perspiration (third day). Lying on right side, with saliva flowing out of right corner of mouth, on waking at 7. 45 p.m (eighteenth day). Woke lying partly on back, partly on right side, with hands placed flat on chest the right being higher up than the left and left leg partly drawn up (fourteenth night); Woke on back, with arms placed horizontally across chest (the left arm being highest up), at 4 p.m (sixteenth day); woke three times on back, position of hands not noticed (seventeenth day); on back, with arms folded and entwined across chest (eighteenth day); on back with hands under back of head, the palms being directed towards the head and the right hand between the left hand and head; afterwards on back, inclining to right side, right hand under back of head as before; afterwards on back, with arms folded across chest, the right arm resting upon the left in a vertical direction (nineteenth day); woke on back, position of hands not noticed (twentieth and twenty- second days); on back, hands flat on body (twenty-third day); on back (twenty-fourth, twenty-fifth, body (twenty-third day); on back (twenty fourth, twenty fifth, twenty sixth, and twenty eighth days); on back, arms lying across body transversely, the right being highest up (twenty ninth day). Various dreams, among others that I had epileptic fits (tenth night).

Fever

 Chilliness. Feeling of coldness of both external ears, this lasted a few seconds, then suddenly they became warm again (after fifteen minutes).. сold feeling beginning at back of cervical vertebrae, coming down spine as far as lower dorsal vertebrae, then it spread over back causing shivering; then in a few seconds feeling of coldness at both elbows, it felt like a douche down the back (all this was in a moderately warm room), (after thirty minutes). Heat. Heat of face, especially left side, on entering warm room, during the toothache (fourth day). Sweat. Slight perspiration, on waking (third and sixteenth days). Warm sweat broke out all over him, even on his head, directly he began to walk in the morning, for two days (third day). вroke out in a warm sweat all over, feeling as if in a hot air bath; warm sweat stood in drops on forehead, when sitting indoors (after three hours, first day); broke out again in a warm sweat, when sitting indoors, at 1 P. M. (third day); again for ten minutes, at 2. 30 P. M. and 5 P. M. (fourth day).

Acute conditions

 Aggravation, (Afternoon), Twitching in eyelid. Aggravation, (Evening), Pin in face; on entering warm room, pain in teeth. Aggravation, (Bending head down), Pain in side of vertex. Aggravation, (Cold outdoor air), Pain in face; pain in teeth. Aggravation, (When expiring), While lying on left side in bed, sensation in left lung. Aggravation, (Deep inspiration), Shooting in side of chest. Aggravation, (Laughing), Pain in face. Aggravation, (Lying on painful side), Pain in teeth. Aggravation, (Lying on right side), Pain from below scapula into chest. Aggravation, (When sitting bent over table), Pain between the shoulders, etc.

Modalities
