Гиперчувствительность - патологическое явление, связанное с усилением восприятия раздражителей обычной силы (слуховых, зрительных, обонятельных, тактильных и др. Сопровождается физическим и психическим дискомфортом, иногда - болью. Диагностика включает консультации врачей-специалистов, визуализирующие исследования (МРТ, эндоскопия) и электрофизиологические исследования. В зависимости от формы гиперчувствительности и выявленных причин назначается соответствующая терапия.

Гиперчувствительность (повышенная чувствительность, гиперестезия) основана на искажении восприятия и проведения нервного импульса по рефлекторной дуге. В результате возникают чрезмерные ощущения, не сравнимые по силе с раздражителем. Воспалительные, аллергические процессы, заболевания центральной и периферической нервной системы, психогенные расстройства могут изменять чувствительность различных органов. Фотофобия (светобоязнь) - патологическая и болезненная реакция на световые раздражители нормальной интенсивности, такие как обычный дневной свет или рассеянное искусственное освещение. При этом возникает желание закрыть глаза, закрыть глаза непрозрачной повязкой, надеть солнцезащитные очки. Причинами повышенной чувствительности к свету являются:
• Глазная патология. Конъюнктивит, кератоконъюнктивит, иридоциклит, увеит, глаукома, синдром сухого глаза.
• Кефалгия. Головная боль напряжения, лекарственная головная боль (чрезмерное употребление), мигрень.
• Патология центральной нервной системы. Менингит, энцефалит, инсульт.
• Вирусные инфекции. ОРВИ, корь, грипп.
При жалобах на повышенную чувствительность к свету рекомендуется посетить офтальмолога, пройти комплексную диагностику органа зрения и зрительных функций (визометрия, биомикроскопия, измерение ВГД, проба Ширмера). Если причины светобоязни обусловлены патологией ЦНС, проводят МРТ головного мозга, МР-ангиографию. До исчезновения симптома рекомендуется использовать режим затемнения, снизить зрительную нагрузку. Для лечения заболеваний глаз подбирают капли, мази, проводят внутриглазные инъекции.
Пониженный порог восприятия звуковых раздражителей называется гиперакузией. Болезненное восприятие звуков может быть вызвано привычными шумами нормальной громкости: разговором, телефонным звонком, звуком бытовых приборов и Явления гиперакузии могут быть симптомом следующих состояний:
• Поражение среднего и внутреннего уха. Отосклероз, акутравма, болезнь Меньера, перилимфатический свищ.

К неврологическим методам диагностики относятся МРТ головного мозга, ЭФИ, люмбальная пункция с анализом ликвора. Лечение проводят с использованием консервативных (медикаментозная терапия, физиотерапия) и оперативных методов (декомпрессионные операции внутреннего уха, пластика нервов и ), психотерапии.
Острая болевая чувствительность развивается при повышенной возбудимости болевых рецепторов или проводящих путей, передающих нервные импульсы в ЦНС. Гипералгезия может быть первичной (при активации болевых рецепторов в очаге поражения) и вторичной (при избыточном раздражении центров, воспринимающих болевой сигнал). Причинами повышенной чувствительности к боли являются:
• Ожоги. Термический, химический, солнечный, фитофотодерматит.
• Кожные инфекции. Рожа.
• Заболевания центральной нервной системы. Инсульт, ЧМТ, мигрень, демиелинизирующие патологии.
• Невралгия. Опоясывающий лишай, невралгия тройничного нерва.
• Другие причины. Авитаминоз, прием опиоидных обезболивающих, фибромиалгия.
Повышенная чувствительность полового члена может быть врожденной особенностью мужчины, возникать в подростковом возрасте, вследствие обрезания крайней плоти, при надавливании или раздражении половых органов необоснованно подобранным бельем. Это состояние часто сопровождается преждевременной эякуляцией, болью при прикосновении к головке, дизурическими расстройствами. К патологическим причинам относятся:
• Воспалительные заболевания. Воспаление головки полового члена, грибковые воспаления головки полового члена, герпетической этиологии, при венерических заболеваниях.
• Другие мужские заболевания. Фимоз, короткая уздечка.
• Дерматозы. Атопический контактный дерматит, генитальная экзема.
Причины повышенной чувствительности головки полового члена устанавливает врач-андролог или дерматовенеролог при приеме и осмотре. Дополнительно может потребоваться проведение анализов на ИППП, аллергопробы, после чего назначается местное и общее лечение (санация препуциального мешка, орошение антисептиками, нанесение мазей и кремов, противогрибковых, противовирусных, антибактериальных, антигистаминных средств).
